料金は4,400円(税込)です。

在庫管理の関係上、予約方法は電話または、受付窓口にて承ります。

Web予約やe-mail、Googleのメッセージ等での予約は行っておりませんのでご注意ください。

予約状況やワクチンの入荷状況によっては、お待ちいただくことがございますので、ご了承ください。

健保組合の利用券をお持ちの場合には、必ず保険証と利用券を予めご用意の上、ご来院ください。

両方のご提示がないと利用券でのワクチン接種をお受けできません。

また、予め「インフルエンザ予防接種予診票」をご記入いただき受診することも可能です。（下記よりダウンロードできます。）

ワクチン接種当日の来院前に記載の上、受診をお願い致します。

Flu shot season is coming.

We start providing a vaccination form October 3rd(Tue). You can make an appointment on telephone. Vaccination appointments are not accepted by web reservation, email, SNS messages such as Google, etc. Sometime, supplier won’t be able to catch up the number of vaccines, in the case, we need to arrange the schedule. You can also take vaccination without an appointment, if we have enough vaccines.
If you have a coupon from the Health insurance Society, be sure to prepare your insurance card and coupon in advance .

If you don't have an insurance card and coupon, you will not be able to be vaccinated with a coupon.

Credit card is not available for any coupons or discount fee.
